A requirement becomes an Epic, Features, Stories and Tasks, each with testable Given/When/Then acceptance criteria, assumptions, risks and sequencing.

A human decides. The submitter can never approve their own proposal, and every decision is recorded with who made it and when.

Evidence is produced as a by-product of normal work — the plan, the decision, the trace and the change plan, ready for audit rather than reconstructed for it.
